PATH - Projects for Assistance in Transition from Homelessness

The PATH Team provides street outreach services in Oakland and Macomb counties through a grant from Michigan Department of Health and Human Services (MDHHS) through funding from SAMHSA (Substance Abuse and Mental Health Administration) This program works with those who are experiencing on-the-street homelessness, by canvassing the community through outreach, to locate the most vulnerable who need to be connected with housing as well as resources and supports.
